Understanding Blood Pressure Categories

Blood pressure is the force of your blood pushing against the walls of your blood vessels. It is measured in millimeters of mercury (mmHg) and recorded as two numbers. The top number (systolic) measures the pressure in your arteries when your heart beats. The bottom number (diastolic) measures the pressure in your arteries when your heart rests between beats.

The AHA/ACC 2017 Guidelines

In 2017, the American College of Cardiology and the American Heart Association released updated guidelines for the prevention, detection, evaluation, and management of high blood pressure in adults. The categories are:

  • Normal: Systolic less than 120 AND diastolic less than 80.
  • Elevated: Systolic 120-129 AND diastolic less than 80. Lifestyle changes are recommended to prevent progression to hypertension.
  • Stage 1 Hypertension: Systolic 130-139 OR diastolic 80-89. Depending on your cardiovascular risk, doctors may recommend lifestyle changes alone or with medication.
  • Stage 2 Hypertension: Systolic 140 or higher OR diastolic 90 or higher. Medication and lifestyle changes are typically required.
  • Hypertensive Crisis: Systolic over 180 AND/OR diastolic over 120. This requires immediate medical attention.

Why High Blood Pressure is Dangerous

Often called the "silent killer," high blood pressure usually has no symptoms. If left uncontrolled, it can lead to severe health consequences including heart attacks, strokes, chronic kidney disease, heart failure, and vision loss. Managing blood pressure through a low-sodium diet, regular exercise, weight management, and medication (if prescribed) is crucial for long-term health.

Frequently Asked Questions

What do the two numbers mean?

The top number (systolic) shows arterial pressure when the heart beats. The bottom number (diastolic) shows arterial pressure when the heart rests between beats.

Which number is more important?

Both are important, but for people over 50, systolic pressure is often given more attention as a major risk factor for cardiovascular disease.

Can anxiety cause high blood pressure readings?

Yes, stress and anxiety can cause temporary spikes in blood pressure. "White coat hypertension" happens when a patient’s blood pressure is higher at the doctor’s office than at home.

What should I do if my reading is in the Hypertensive Crisis range?

Wait five minutes and test again. If it is still over 180/120, seek emergency medical care immediately, especially if experiencing chest pain, shortness of breath, or numbness.

Can lifestyle changes really lower blood pressure?

Absolutely. Weight loss, a DASH diet, reducing sodium, limiting alcohol, and regular aerobic exercise can significantly lower blood pressure numbers.
