The estimated Glomerular Filtration Rate (eGFR) is widely considered the best overall index of kidney function in health and disease. Your kidneys filter waste and excess fluids from your blood, which are then excreted in your urine. The eGFR calculation provides an estimate of how well your kidneys are performing this crucial filtration process.

What is the CKD-EPI 2021 Formula?

In 2021, the CKD-EPI (Chronic Kidney Disease Epidemiology Collaboration) introduced a new equation to estimate GFR. This updated formula uses your age, biological sex, and serum creatinine levels. Importantly, it was designed to be race-free, addressing concerns about racial disparities and potential delays in treatment that arose from earlier formulas that included a race coefficient.

Why Race Was Removed from eGFR Calculations

Historically, eGFR formulas (such as the MDRD and older CKD-EPI formulas) included a modifier for individuals who identified as Black. This was based on flawed assumptions about muscle mass differences between races. The inclusion of this modifier often resulted in higher eGFR estimates for Black patients, which could mask the severity of kidney disease and delay crucial interventions, such as referrals to nephrologists or placement on transplant waiting lists.

The adoption of the race-free 2021 CKD-EPI equation represents a significant step toward equity in kidney care, ensuring that all patients are evaluated using the same standard.

Interpreting Your Results: GFR Categories

The KDIGO (Kidney Disease: Improving Global Outcomes) guidelines classify chronic kidney disease into five stages based on eGFR:

  • G1 (eGFR ≥ 90): Normal or high kidney function. (CKD is only diagnosed if other signs of kidney damage, like proteinuria, are present).
  • G2 (eGFR 60-89): Mildly decreased kidney function.
  • G3a (eGFR 45-59): Mild to moderately decreased kidney function.

Limitations of eGFR

While eGFR is a valuable tool, it has limitations. It is an estimate and may not be accurate for everyone. For instance, eGFR may not be reliable for individuals with extreme muscle mass (such as bodybuilders or those with severe muscle wasting), pregnant women, or individuals following a strict vegetarian or vegan diet. In these cases, other tests, such as a 24-hour urine collection for creatinine clearance or a cystatin C test, might be more appropriate.

When to Test

Your doctor will likely order an eGFR test as part of a routine comprehensive metabolic panel (CMP) or basic metabolic panel (BMP). You may need more frequent testing if you have risk factors for kidney disease, such as diabetes, high blood pressure, a family history of kidney failure, or cardiovascular disease.
